Dear forum users,
Does anyone out there have a noise and vibration questionaire that I will be able to issue to my field engineers to determine whether or not I have a significant problem with noise and vibration.

If you see the HSe ACOP on the subject it includes a good form. L140 Hand-arm Vibration Control of Vibration Regs 2005.pdf It's sister guide on whole body aslo has one but this tends to be less of a problem in most areas.
INDG 362 availible free on the HSE website will give you a good hint as to what needs to be included in your form.
