Under LOLER and RIDDOR if you are performing a lifting operation and it fails, in this case an eyebolt sheared (machine at no height at this stage)is it reportable?
x4 slings into x4 eyebolts, 2tonne weight.
I think Yes but some say No.

RIDDOR Schedule 2, 1. the collapse of, the overturning of, or the failure of any load-bearing part of any -
Lift or hoist
crane or derrick
MEWP
Access cradle etc
excavator
pile driving
FLT
My interpretation is that the eye bolt is a load bearing part of the lifting equipment and it failed, therefore reportable under RIDDOR.

Steve

I'm not going to pretend I'm an expert, but I would have thought that if the part that failed is equipment which has been tested and has a test certificate then it is reportable.
