Posted By Alan E McKerns Under LOLER and RIDDOR if you are performing a lifting operation and it fails, in this case an eyebolt sheared (machine at no height at this stage)is it reportable? x4 slings into x4 eyebolts, 2tonne weight. I think Yes but some say No.

Posted By terryt I'm not going to pretend I'm an expert, but I would have thought that if the part that failed is equipment which has been tested and has a test certificate then it is reportable.
