It depends on the model. The problem with most models is that they assume only one guilty party and fail to address "contributors".

Hang on, I did the TOPSET method and they have a section for People - includes 'guilty' party, witnesses, managers, supervisors and 'Others'.

Richie, sounds familier. Blew up the CO's landrover by mistake, honest !! Heard the same sort of comment at the review.

Hi Tony

Adrian didnt do my class one, but he was wheeled out now and again when we needed to taught something difficult, like accident causation models....

My thoughts - the number of different models available, means you can pick one that suiyts your particular circumstances.
