I need to start a regime where DO's are reported. I plan to have a post box and post card system to enable guys on site to report their concerns and obervations anon. Has anybody got a template by which to so this or any thoughts.

Do you really mean "Near Misses"?

In the past I have made the forms freely available to all (eg in canteens etc) and anyone could submit one - they could either go through the supervisor or to the safety Dept.

If safety professionals are using the wrong terminology it's no wonder that many in the workforce think every incident is a 'dangerous occurrence'

Okay. Near misses, dangerous occurences, safety concerns, event witness statement. I'm sure there are other terms. Has anybody got a form i may adapt for the reporting of such events

Sure gary was only making reference to the fact that some D'Os are RIDDOR reportable.

Assuming you are in the UK, I would go as far as saying all DO's are reportable under RIDDOR - as RIDDOR has defined what a DO is.

The only reportable D'Os are defined in Riddor.

A "dangerous occurence" may occur however it may not be necessarily RIDDOR.

Doesn't this thread just sum up this forum.

It is perfectly obvious what toecap is asking for, yet (some) people feel the need to patronise, belittle or be just plain rude.

I think this says far more about safety professionals than an apparent 'lack' of knowledge.

Toecap

There was a thread on this last week which might be helpful. It's slipped onto page 2 now - loook for "near miss postcards" - one or two people on there had systems they were willing to share.

It is really important to get the terminology right - it is not trivial at all.

In my view, and considerable experience, the terms "Dangerous Occurrence" and "Near Miss" are very different and so should be kept separate and used in context. The possibiliity of getting confused with the RIDDOR NDO is obvious.

In addition, it is certainily not true that all DOs are NDOs under RIDDOR!

RIDDOR is very specific on what is reportable.

I find there is more of an issue with the term "near miss". It's hard to get someone to understand how a FLT hitting a wall can be a near miss when obviously it was actually a hit! We now call them near misses/incidents and have changed the name of our form accordingly.

I too would avoid the term dangerous occurrence though I don't think it's that much of a big deal provided the person responsible for RIDDOR reporting understands the difference...

Toecap i will mail you direct about the post card.

I have to say on this post that no wonder some people worry about asking questions as they are in fear of being shot down.

I would say an FLT hitting a wall is a direct hit!!!... especially if I was the wall!! ;-)

On a more serious note, we tend to refer to a Dangerous Occurance as serious incident, that may have involved equipment or property damage, but not human injury.

We class a near-miss as an non damage event, but only by luck, chance or co-incidentce it did not result in an injury accident or dangerous occurance.
