Rank: Guest		Posted By The toecap I need to start a regime where DO's are reported. I plan to have a post box and post card system to enable guys on site to report their concerns and obervations anon. Has anybody got a template by which to so this or any thoughts.

Rank: Guest		Posted By garyh Do you really mean "Near Misses"? In the past I have made the forms freely available to all (eg in canteens etc) and anyone could submit one - they could either go through the supervisor or to the safety Dept.

In my view, and considerable experience, the terms "Dangerous Occurrence" and "Near Miss" are very different and so should be kept separate and used in context. The possibiliity of getting confused with the RIDDOR NDO is obvious. In addition, it is certainily not true that all DOs are NDOs under RIDDOR! RIDDOR is very specific on what is reportable.

It's hard to get someone to understand how a FLT hitting a wall can be a near miss when obviously it was actually a hit! We now call them near misses/incidents and have changed the name of our form accordingly. I too would avoid the term dangerous occurrence though I don't think it's that much of a big deal provided the person responsible for RIDDOR reporting understands the difference...

Rank: Guest		Posted By Paul Darby I would say an FLT hitting a wall is a direct hit!!!... especially if I was the wall!! ;-) On a more serious note, we tend to refer to a Dangerous Occurance as serious incident, that may have involved equipment or property damage, but not human injury. We class a near-miss as an non damage event, but only by luck, chance or co-incidentce it did not result in an injury accident or dangerous occurance.
