I need to confirm the situations where faults/recommended actions identified by a statutory inspection would be notifiable to the HSE (I believe COMAH sites are one example)

morning Sharon,
Following the letter to the N'th degree, and being the founder of a testing company we are suppoosed to inform HSE of every failure - highly impractical! However,Reporting all failures does help to find trends of equipment type failure (rigging screws with two left handed threads being a typical example). but i'm not sure that the HSE would appreciate the extra work, I can and do fail dozens of pieces of equipment every week. Dangerous Lifting practices, Misuse and poor environments are always reported though.
