Posted By Sharon Wooller I need to confirm the situations where faults/recommended actions identified by a statutory inspection would be notifiable to the HSE (I believe COMAH sites are one example)

Posted By stone rose morning Sharon, Following the letter to the N'th degree, and being the founder of a testing company we are suppoosed to inform HSE of every failure - highly impractical! However,Reporting all failures does help to find trends of equipment type failure (rigging screws with two left handed threads being a typical example). but i'm not sure that the HSE would appreciate the extra work, I can and do fail dozens of pieces of equipment every week. Dangerous Lifting practices, Misuse and poor environments are always reported though.
