Does anyone have any good suggestions for what should be covered on a health and safety meeting between supervisors and a QHSE Manager? I know it is a bit of an open ended question but I have posed the question in such a way as to draw answers which may not be the norm. Any contribution is greatly appreciated.

Incidents reported and closed out/not closed out Near misses- lessons learned Audits/inspections/surveys- actions carried out PPE surveys- compliance noted Training- feedback from employees Are all staff ticketed/ have appropriate training? Tool box talks/H&S issues raised/discussed by staff Risk assessments completed/reviewed Feedback from Health and Safety Culture surveys Is the PTW system working? Any disciplinary matters relating to H&S? Etc etc

In addition to the above you could include: Understanding of the H&S Policy Identification of first aiders & fire wardens Supervisors H&S responsibilities Leading by example Recommendations for improvement Feedback from the employeeEmergency response prepardness.

Another topic that is becoming more and more present (certainly in my industry) is management of change (MOC). I am currently developing a procedure for this as quite often when changes occur (personnel, equipment, procedures etc) the risks aren't managed correctly and the liklihood of an incident increases.
