Scenario
Split pin and washer come off the Anchor point for a wire rope. Rope remains in position, no loss of load but a potential for the rope to slip off the anchor point.
Is this a failure of a load bearing part?

Don't think so, nothing actually failed did it.
Will be interesting to hear others views though.

My understanding would be - the split pin is not a load bearing part - it is there only to retain the load bearing part. So loss of the split pin / failure to spot the absence of the split pin is a serious defect but not reportable under RIDDOR.

Agree, the split pin is a locking device, not load bearing. How does a properly fitted (bent legged) split pin come out - provided the legs have been bent correctly? Assume a conventional split pin? Like this http://www.ajreeves.com/132-split-pins-9253-p.aspBeen used a few times/legs bent straight again?

Thought I'd bring a bit of harmony to the community with something we can all agree on.
Having examined the item further the load would not have been affected (due to design and redundancy in the equipment) and their was no failure of any item.
All that said all similar hoists were taken out of use and have been checked (no issues) and a root cause analysis is being carried out.

Amazingly no one has said "report it to be on the safe side". IMHO and considerable experience you should NEVER do this - only report when you are certain.
