Hi
I'm looking to overhaul our current Accident/Incident reporting database and was looking for recommendations of systems/software you are currently using.
We're currently using a bespoke system which is part of our management information system. The initial design wasn't ideal (despite me providing a detailed spec) and it's become cumbersome and not particularly user friendly.
Ideally I'm looking for something that's 'off the shelf' but with scope for some company-specific adaptations. Also what would be great is something that comes with (or has the capacity to engage with) a smartphone app for reporting.
Your suggestions would be appreciated.

I am surprised there has not been a flood of responses to this question.
That said I am not going to be able to give you any recommendations myself - we use an inhouse, bespoke system and are able to get it tweaked/amended as needed.
We recently looked at bringing an "off the shelf" package in but very few were able to meet our needs unless there were some potentially costly tweaks made.
What I did find is that in the main you will find a safety management system that runs a core module and then you would add on the accident reporting module, or risk assessment module etc.
Some of these systems will be very expensive whilst others are more appropriate for the smaller budget. Perhaps you could let us know in what type of environment you are looking to make the system work - I note you are a member of the healthcare group, perhaps a bigger hint might draw recommendations from others in a similar field.
Before we suffered budget reductions, I had looked at a system called EOA Safety Organiser - it seemed to be pretty good and there was an option to visit a "sample site" and see how it all worked. The company seemed to be happy to apply minor tweaks in certain circumstances.
We had also thought about linking tablets to the system, but it was really only available in the more expensive systems - that may have changed (some 2 years on)

There have been numerous threads on this over the last year so is worth a search. There are many companies out there and offer similar products, it really depends on the cost and the way you manage H&S and complexity as to which will be best. We have just moved from one system to another as we wanted more that just accident management and it was the most cost effective solution and I had real issues with the previous system. make sure you know what you want to put in and get out of the system so that you can test it correctly otherwise you could end up with a white elephant.
