I am particularly interested in implementation plans, how they clarified roles and responsibilities, training / briefing arrangements, monitoring, recording, auditing etc.,

It gives some good examples and is the stuff I use for most of my training. I have available some forms you could use and always willing to find somebody in our organisation to give advice.
