I have been asked to format a risk assessment and permit to work form which applies specifically to live working on fixed electrical installations, does anyone out there have such a thing or know were I could find any information?
Thanks for the help

You will be hard pushed to justify any live working and comply with the EWA. You should try not to allow it apart from a bit of commissioning.

As stated see the other thread on this subject.
A PTW is used as confirmation that a circuit is DEAD, and should only be used for confirming isolation and other safety precautions taken to ensure that a circuit REMAINS DEAD during the course of the work.
A Sanction for Test is used for live working, and is subject to the stringent requirements of Reg 14 of the Electricity at Work Regs, detailed in the other thread.
