Does anyone know the name of a training video that was doing the rounds in the 90's which featured an american guy telling his personal accident story. He had been working in a oil/gas plant and had gone to close a valve bypassing many of the safety controls and ended up causing a fire where he received severe burns. It lasts about 45 minutes and goes into his treatment and the effects on his family.
I think it was called something like "Whatever happened to (insert name here)". I can't find it anywhere and I am beginning to question myself as to whether I imagined it!

This sounds like a scenario from a typical american-made safety film. Try asking one of the US based safety web sites if they can help

Remember Charlie was published by TVA in Harrogate. Their website at www.tva.co.uk says it is still available for £499 +VAT.
