Again thank you in advance for your comment/help.
I am trying to develop a safe system of work with regards to witnessing, testing and powering up a substation, after a (near miss happened were by x3 HV leads were not connected and it was only by chance it was noticed before the powering went ahead) this substation was going to be carrying 11000 volts, a fair old belt if this was not noticed.
I was wondering if anyone would have a safe system of work I could look at or point me in the right direction.

Posted By Paul Clarke-Scholes MIOSH You might be better off with a good old fashioned quality check sheet (call it a permit to power up if you wish). Something along the lines of "I the installation foreman have checked that all the cables have been connected and that the juice can flow". Amazing how making someone put a signature on a piece of paper makes them actually do the double check. For extra safety get a countersignature.

Posted By Paul Darby We had a similar start up sheet when I used to test rocket engines. We got a small team of experienced people together and formulated a signature check sheet that formed the "go" to fire.
We started with a blank piece of paper and after an hour of discussion and simulating a firing we eventually had over 30 checks on the sheet.
Then the station superintendant was the final sign off. It worked really well and never came close to letting us down.
