Dangerous Occurrence - pressure system- RIDDOR?

Posted By Mike Nixon If a metal compressed air pipe comes off a wall with a loud bang not long after a new system has been commissioned is this reportable under RIDDOR? No one near the incident at the time and bracket went onto floor due to insufficient screws in place. If answer is Yes is it still worthwhile reporting incident after 3 months?

If it were me, I give the HSE a call to let them know of the situation, that it was three months ago, and what you have done about the issue since, and then take it from there.

Unless the pipe actually fractured or collapsed I would say it is not reportable.
From what you say, it was just that the mounting bracket fell off.
