If a metal compressed air pipe comes off a wall with a loud bang not long after a new system has been commissioned is this reportable under RIDDOR?
No one near the incident at the time and bracket went onto floor due to insufficient screws in place.
If answer is Yes is it still worthwhile reporting incident after 3 months?

Hi Mike, one of the reportable issues under RIDDOR as a Dangerous Occurance is:

Explosion, collapse or bursting of any closed vessel or associated pipework;

From the info you have given, I think that would be reportable. If it were me, I give the HSE a call to let them know of the situation, that it was three months ago, and what you have done about the issue since, and then take it from there.

Unless the pipe actually fractured or collapsed I would say it is not reportable.

From what you say, it was just that the mounting bracket fell off.
