Formal system for raising H&S issues to Director level

Has anyone any comments/suggestions on how to set up a robust system for reporting all H&S issues to our Directors in a timely fashion.
They are notoriously difficult to meet face to face and like daily/weekly reports depending on the severity of the issue.
Even better if anyone has a template/strategy document for me to view; I would be very grateful?

There lies your problem - daily/weekly reports tend to get 'filed' - initially they do get read, but after time the 'reading' gets shorter and the 'filing' gets quicker. No template is going to prevent this
For urgent matters, nothing beats walking into the bosses office and saying "you know my job is to keep you out of jail, well .........."
I have always found this most effective [and whatever your job description says - your boss really does think you are there to keep him out of jail, even if he doesn't admit it]

I agree; knocking on the door is the only effective method and templates are a very poor way of communicating.
However, a paper record is critical to prove that they have been notified?

Interesting to find out how H&S issues are put onto the board meeting minutes... that is a good way to confirm that it actually happens (another get out of jail system)
if so how is the information passed by who and who is assigned to action?
At the end of the day the safety management structure should have this in place if 1 form or another?

I have found that for major issues knock on the door and tell 'em direct.
For routine issues work through the chain of command to get a weekly or monthly reporting route. Perhaps get H&S a standing agenda item for management meetings and ensure you are invited to them.

I agree with the knock on the door methodology but you often still get the in one ear out of the other scenario. I find it best to mention it them, then have it as an agenda item or discussion point at the next meeting. Therefore it is in writing (A bit of back covering exercise).
If requirements are not acted upon then a non conformance / non compliance report is raised; this ramps it up slightly as justification of action or non action is required to be recorded.
I have this process written ito thwe OHSMS manual, which all managerial personnel are required to read and sign that they have understood the contents.
Obviuosly the above is very much dependant upon the urgency of the situation.
